Which version of ReportBuilder are you using? In an earlier version of the PDF device (pre RB 10.x), each image was saved to a file before being written to the PDF file. This was to save memory however it caused problems with the Server edition of ReportBuilder so it was removed. The latest version of ReportBuilder does not create any temporary files when exporting to PDF.
What does your report consist of? Perhaps I can try to recreate this behavior on my machine. I did a quick search again through the PDF device code and still do not believe any temporary files are created during the generation process. Are you able to see this behavior with a minimal example?

If that is true, especially if the file names are similar, I would bet it is
something in your code because they are completely different drivers
