[] BFR
[] TX Duration
[] Other

Fluid/ Cardiac
[] Inability to control BP
[] BP > ---/-- [] BP < --/--
[] Inability to control fluid/na+ intake
[] Inability to obtain hemostasis post tx
[] r/t excessive fluid removal
[] poor cardiac status
[] Non-compliance with medications regime

[] Na+ Modeling
[] Fluid Modeling
[] Adjust medications and/or schedule
[] Instruct/review
[] fluid restriction
[] na+ restriction
[] Other

Nutritional Status
[] Uncontrolled electrolyte balance
[] Uncontrolled mineral balance
[] Uncontrolled cholesterol/lipid
[] Wt gain
[] Wt loss
[] Uncontrolled blood sugars

[] Adjust medication
[] Instruct/review
[] high/low phos foods
[] high/low K+ foods
[] low fat/chol/trig diet
[] HBV protein diet
[] Concentrated CHO diet
[] Kcal restricted diet
[] Diabetic Education Referral
[] Provide written materials
[] Protein/caloric supplement

But when I view it in the report, the formatting gets messed up. See below.
I am currently using Report Builder Version 6.03.

ReportBuilder relies on TRichEdit. Try showing this rtf in a TRichEdit on a
TForm without RB involved. What does it look like?

See the rich text articles in the tech tips newsgroup for info on replacing
the richtext engine in RB with Infopower or WPTools. You can also switch the
version of the RichEd dll to see if that helps. These are all described in
    the articles.
