When I load a report from a file, the PrintToFile property appears to be
False. How is that set/saved in the report? If I set PrintToFile:=True
right after loading the report, it works fine.

It is saved in the rtm file. The AllowPrintToFile is saved as part of this
file.

If the template is saved as ascii and not binary (see the template property
on the report component) you can see the property value which is saved.
You'll see something like this for an empty report if you open the rtm:
