If you can, try printing them to an archive format first then the PDF format, this may help narrow down the cause. You will have to determine if it is inside RB or in TExtraDevices.
If you have CodeSite, you might want to send the TppReport object for viewing just before printing.
Also, as this has to do with TExtraDevices, I would send James Waler a note and see if he has seen anything like this.

What version of RB? TExtraDev? Delphi? Can you reproduce this without
sending it to a PDF? Just printing to a PDF reproduces it?
